Application for Use of Hmong New Hope Alliance Church Properties

 

This reservation form is intended to inform the Governance Board of Hmong New Hope Alliance Church of your request to reserve and utilize the physical amenities of the church. The information you provide helps the board review your request, ensure proper coordination of facility use, and maintain an orderly schedule for all church-related and community activities.

Qualifications, Requirements and Restrictions for the Use of Hmong New Hope Alliance Church Properties

For Church Members and Non-Members

RULES AND REGULATIONS

A. CHURCH PROPERTY RENTAL GUIDELINES AND PROCEDURES

  1. Church properties allowed for use: - Sanctuary, Classrooms, Fellowship Hall, Church Field and Kitchen
  2. Both Church members and non-members may use any of the church facilities for unrelated church activities with prior approval from the Senior Pastor, Head Elder and Secretary.
  3. A list of events that are allowed include, but are not limited to: - Graduations, Meetings, Conferences, Wedding Ceremonies, Personal Celebrations, etc.
  4. Events that will NOT be allowed on any of the church properties include, but are not limited to: - Dance parties, shamanistic teachings or ceremonies, any parities involving alcohol usage, bowing down to anyone other than God, or actions that plainly goes against Christian teachings
  5. Hmong New Hope Alliance Church facilities are NOT available to for-profit businesses, or to organizations for use as location to raise funds, or for games of chance
  6. The use of the church properties shall NOT be permitted for use on Sunday, except with special approval of use by the Board of Governance.
  7. No use is to be made of Hmong New Hope Alliance Church during or near the time of worship.
  8. For usage of the church field, the applicant must have liability insurance with a minimum coverage of one million dollars and submit proof of the liability insurance to the Board of Governance at least two weeks prior to the usage of the church field.
  9. The applicant must be present during the entire usage agreement and is liable for all of his/her guests’ actions.
  10. All applications must be submitted a month prior to the event.
  11. All applications must be approved by the Senior Pastor, Head Elder and Secretary before any usage of any of the church properties shall take place.
  12. All fees much be paid in full two weeks prior to the event, if no payment has been made 2 weeks prior, the event may be cancelled at the choosing of the Board of Governance.
  13. Non-members of the church are required to have $300.00 safety deposit which is excluded from the fees for the use of the church properties. The safely deposit fee must be made at the same time at which the fees for usage are due. The safety deposit will be returned to the applicant provided all facilities are well cleaned, no equipment was damaged by the applicant and his/her guests, and all rules and regulations as set by the church have been followed during the use of the church properties.

B. SETTING UP FOR THE EVENT

  1. The applicant has two days to set up for his/her event. Scheduling set up days must be made with the Head Elder of the church
  2. Responsibilities of setting up tables and chairs and returning them to storage areas is that of the applicant and his/her organization
  3. All scenery or props must be free-standing. No nails, screws or stage hooks may be used anywhere within the church facilities.
  4. There shall be no moving of church items/décor without first notifying the Head Elder of the church.

C. REGULATIONS DURING THE EVENT AND CLEAN-UP REGULATIONS

  1. There shall be no firearms, drugs, alcohol, tobacco, smoking or animals (except guide dogs) within the church properties by anyone whether the applicant or one of the guests of the applicant.
  2. Parking in lawn areas is prohibited. *except for overflow
  3. Food and drinks are only permitted inside the Fellowship Hall.
  4. Functions are not to last beyond 10:00pm.
  5. Sound systems of Hmong New Hope Alliance Church can only be handled or be operated by church members approved by the Board of Governance.
  6. Facilities and equipment are to be left in the same condition as they were found: windows closed, floors clean of debris, lights off, playground and lavatories picked up, etc.
  7. Missing, damaged or broken items shall be replaced and/or repaired by the applicant or using organizations.
  8. Bags of trash and other debris shall be removed to the trash bin by the applicant or using organization

FEES

Members

Members of Hmong New Hope Alliance Church have the priviliege to rent the church facility at no cost.

The total amount will amount to $0.

Non-Members

Non-members of Hmong New Hope Alliance Church have the privilege of renting the church facility at the cost listed in the sections below.

Renting all items excluding the church field will grant a grand total of $1,000.

Renting all items will grant a grand total of $1,300. Note that if you sum the total of items individually, you will not get $1,300. There is a discount to renting all items.

All non-members are required to put down a $300 security deposit.

Safety Deposit shall be returned at the applicant’s address provided all rules and regulations have been met.
